Transition Hub

“Home READY, School READY, Child READY”

The Transition Hub, delivered by Achieving for Children (AfC) Virtual School, offers a specialised educational and wellbeing support programme. It is designed to meet the unique needs of children and young people who are in care, newly entering care, or experiencing transitions in school or care placements. The Hub also supports those on a Child Protection (CP) or Child in Need (CiN) plan with school attendance rates between 80%–89.9%.

Who We Support

The programme is available to students in Years 1 to 11 in mainstream educational settings, with or without an Education, Health and Care Plan (EHCP). It is a co-educational initiative and aims to ensure that every child is:

  • Home READY – supported in their home environment

  • School READY – prepared to engage meaningfully with their education

  • Child READY – emotionally and socially equipped for everyday challenges

Referrals may be made by the student’s social worker or Designated Safeguarding Lead (DSL) when concerns about attendance arise.

Our Approach

The 12-week personalised Education Engagement and Wellbeing programme is built on a strength-based approach and delivered by dedicated learning mentors, Anita Joseph and Sam Kavanagh. Our work is guided by the Six C Model framework and tailored to the specific needs of each young person.

Key elements of support include:

  • Pastoral care

  • Fostering a sense of belonging

  • Enhancing self-esteem

  • Supporting emotional regulation

  • Applying Motivational Interviewing techniques

Collaborative Support

We work in close partnership with:

  • Parents and carers

  • Schools and educators

  • Social workers and key professionals

Together, we create a coordinated support network that reinforces each child’s journey towards improved attendance, wellbeing, and academic engagement.
